You must verify your identity before completing the claim form. To verify your identity, please enter your APEX ID and CLAIM NUMBER. You can find this information in the notice that was sent to you.
If you did not receive a notice or do not have your APEX ID and CLAIM NUMBER, please contact the Settlement Administrator at (800) 355-0700 or claims@apexclassaction.com.
Note: After verifying your identity, you will be redirected to the claim form document to complete and submit your claim.
You may claim reimbursement of up to $5,000 for documented losses, including compensation for up to 4 hours of lost time at $25 per hour.
Required Documentation:
After paying all documented claims, service awards, and legal fees, the remaining funds will be distributed equally among all class members who submit valid claims.
All claims must be submitted by July 9, 2025.
If you don't have documentation, you can still submit a claim and potentially receive payment from the Non-Documented Pour-Over Fund.
Payments will be made after the Court gives final approval to the Settlement at the hearing scheduled for July 24, 2025, and after any appeals are resolved.
